Glacial Tomb in Bloodstained: Ritual of the Night is the final area of the game, and where you’ll face the final three boss fights.
Glacial Tomb only connects to Den of Behemoths.
Below, we’ve included a complete map of Glacial Tomb with a few notes about the area.

To reach Glacial Tomb, you have to be most of the way along the path to the game’s true ending. Defeat Zangetsu in Oriental Sorcery Lab and receive his Zangetsuto, and then head to Hall of Termination. Halfway through your fight with Gebel, attack the moon instead. This will break Gremory’s hold on Gebel. Gremory, as the moon, will then flee to the Garden of Silence. Head there, and attack the moon again to create a portal to Den of Behemoths. Fight your way through Den of Behemoths toward the bottom right to find the path to Glacial Tomb.
Your first stop in Glacial Tomb is near the bottom center of the map. Here, you’ll face Gremory in a boss fight. Defeat him (it?) to receive the Dimension Shift Shard ability — a short range teleport. This will help you reach any items or hidden areas that are closed off like the ones under the stairs in Hall of Termination — and it makes navigating Oriental Sorcery Lab much easier.
After that, head to the left to find Bloodstained’s final two boss fights. Just make sure you’re prepared with potions and meals before you enter the final room. The two fights are (understandably) the toughest in the game, and they happen one after the other without a break in between. You also won’t be able to travel back into the rest of the world once you’re done, so make sure you’ve explored everywhere you want to.
